Transitions… the Process of Change II’s mission is to provide Adult Outpatient Substance Abuse Treatment services to give clients the opportunity to attend treatment and continue living at home and working in the community. By doing this, clients have the time to implement the developed recovery skills and process their experiences in a safe environment, which creates the basic conditions for the establishment of a substance-free life.

We provide a variety of outpatient substance abuse treatment services to those individuals referred by a government entity, an attorney, and even those seeking substance use or abuse assistance on their own.

 

 

Our agency is an approved provider with The Commission on Virginia Alcohol Safety Action Program (VASAP), has an active contract with the Virginia Department of Corrections, and is an approved Substance Abuse Professional with the Virginia Department of Transportation (VDOT) Drug and Alcohol Clearinghouse.

Description of Our Counseling Services

Transitions… the Process of Change II offers ASAM Level 1.0 treatment. As an outpatient substance abuse service provider, Transitions II, determines treatment duration and intensity on an individualized basis. Our treatment is not the “cookie cutter” type and speaks specifically to the client’s individual needs, lifestyle, and stage of readiness for change. Telehealth services are available for those that qualify.

PRE-TRIAL/POST-TRIAL RISK ASSESSMENTS

Comprehensive assessments are conducted using American Society of Addiction Medicine (ASAM) Addiction Severity Index to determine an individual's risk, pre-trial and post-trial, to engage in substance use/abuse.

INDIVIDUALIZED THERAPY

Individual counseling is the process through which clients work in a 1:1 setting with a trained clinical provider. Sessions are held in a safe, confidential setting, which focuses on the individual’s immediate issues and future concerns.

FAMILY/COUPLE INTERVENTION COUNSELING

Family therapy is designed to identify families recognize patterns that contribute to a behaviors that occur within the family. These behaviors include substance abuse, mental illness, and other disorders that impact the functionality of a family. Therapy works to assist family members to break those habits. Family therapy involves discussion and problem-solving sessions with the family alongside of a trained, substance abuse professional.

PSYCHOEDUCATION & INTERVENTION GROUPS

Psychoeducation is the process of providing education and information to clients who are abusing substances or are suffering from a substance use disorder. The goal of these programs to help an individual improve their overall understanding of substance use, triggers, choices and decision making, the stages of change, substance abuse versus dependence, and reintegration into society, within a group setting. As well as applying interventions for continued substance use/abuse despite consequences. Typically, individuals who have a thorough understanding of how to effectively cope, the recognition of internal and external triggers, and demonstrate the ability to identify their areas of strength, will be afforded the opportunity to successfully overcome their challenges.

RELAPSE PREVENTION & RECOVERY

In the relapse prevention and recovery group, clients will work to recognize the early stages of a potential relapse. Clients will also work to understand the process of personal growth with developmental milestones. Tools will be implemented for each client to work to change their life and mold it to their recovery, learn to be completely honest, recognize when to ask for help, practice self-care, and learn to follow the rules of society.

MODIFIED INTEGRATED OUTPATIENT TREATMENT (MIOP)

MIOP is our highest level of treatment with up to 8.5 hours per week of treatment for individuals that require a combination of all our services. In addition the being individualized to the client's needs, a group of professionals work together to ensure the client receives assistance in all areas necessary.

Those with low income are offered financial arrangements based on their ability to afford treatment as a way to promote treatment participation.
